the school year, we encourage you to come to the school to register your
child as soon as possible, even before you move if that is possible.
We understand that adjustment to a new school during the school year is difficult,
but the less your child's school year is disrupted by absence, the better.
Your child should begin attending his/her new school the next school
day after he or she withdraws from the previous school. |
Registration
Requirements
Age: Pre-Kindergarten registration begins March 1st. Pre-K
students must be 4 years old on or before September 1. Kindergarten students must be 5 years old on or
before September 1. First grade students must be 6 years old on or before
September 1. A copy of the child's proof of age must be supplied at the
time of enrollment. For more information on Pre-K registration, please

Immunization: All students in grades Pre-K-12 must provide an
immunization certificate (Georgia DHR Form #3231) at the time of
enrollment. This certificate may be obtained from the county health
department or from a physician upon proof of immunization. A list of
required immunizations may be obtained from the
Georgia
Division of Public Health website.
Ear, Eye, and Dental Examination: All students in grade Pre-K-12
must provide a certificate of ear, eye, and dental examination (Georgia
DHR Form #3300). This certificate may also be obtained from the county
health department or from a physician.
Social Security Number: All students entering this school
system are asked to provide to the school the student's Social Security
number. This number will be used as the child’s student number throughout
his or her years in school. For accuracy, it may be easiest for parents
to bring the original social security card and the school will make a copy.
Parents may choose not to provide this information
by signing a statement indicating that they are declining to provide the
social security number.
Transfer Records: Students transferring into a Muscogee County
school must provide a withdrawal form from the previous school and
official notice from that school to verify grade placement at the time of
withdrawal. The name and address of the previous school also need to be
provided.
Custody: If any person other than a natural parent has
custody of a child, proof of legal custody or guardianship must be
provided before the child may be enrolled in a Muscogee County public school.
Proof of Residence: Proof of residence must be furnished at
the time of registration. Examples of proof of residence are: home
mortgage or warranty deed documents, vehicle tag transfer receipt, utility
bills, bank statements, charge account statements, moving company
receipts, health insurance documents, federal/state tax returns, documents
from a governmental agency, driver's license, and voter registration
information. Proof of residence is subject to investigation. For

When to Register Our school office is open
from 8:00 a.m. to 4:00 p.m. each school day and in the summer. You may come and register your child at any time
during those hours. You need to bring the required documents indicated
above. If your child is transferring from another Georgia public school,
you will need to bring proof of residence and a withdrawal form or report
card to verify grade placement. Many of the other required documents will
be obtained from the previous school.
If your child attended a Georgia private school or
a school
in another state or country, you will need to provide a withdrawal form or
report card, proof of residence and all of the other required documents.

If you're moving to the area during the summer
We strongly encourage you
to come by the school at your earliest convenience to register your children.
We're open every day (Monday through Friday) from 8:00 a.m. to 4:00 p.m.
during the summer as we work to prepare for the new school year.
We would love an opportunity to meet you and your children and to get them
registered at Clubview Elementary School.
Getting your children registered early will allow
us to obtain their records from the previous school, giving us important
information needed to make an effective class placement for your children.
It will also ensure that your children's teachers will be ready to welcome them
at our open house held the week before school begins.
